We start with a oneonone meeting to understand your goals, preferences, current challenges, and the supports already in place.

2. Building a Support Network

We assist with bookings, service agreements, and communication between multiple providers, ensuring everything runs smoothly.

4. Monitoring Progress

We regularly check in with you to see what’s working, what’s not, and adjust your supports as needed.

5. Plan Reviews & Reports

As your plan comes up for review, we help prepare reports, review goals, and advocate for necessary changes or funding.
